The 'to' indicated by made can be time like (40 days) space like (Kyushu)
or more metaphorical ([lit: up to raining]) (contributor: Amatuka)
Note there is some difference between までに and まで.
まで : 'I stayed up till 10pm'
10時まで起きてた。
[You actually went to bed at 10pm]
までに : 'I had to get back by 10pm'
10時までに帰らなくっちゃ
[You could come back earlier, but not later]

Do they ever say 'までは'?
As in,

until 3.'
Or would simple '3時まで' suffice?

3時まで is sufficient.
3時までは is used to emphasis. Please don't dare come to the meeting
room until 3 as we are having an interview. (contributor: Miki)

Further, I believe Madeno is used when modifying a noun like if you used
Made no in example 838 it would translate as: 'I took the plane that goes
